NOTE: You will not be able to fill out and submit these forms on-line, so you will have to print the ones you need and mail them to the proper address. If they are forms to be sent to the NAME Office, you also have the option to fax them (317) 571-8105 if you are using a VISA or Master Card. Please pay attention to the dates and mailing addresses on each form.
