We have several ways you can join or renew.

  1. Click, print, fill out your application, and mail it along with your check or credit card  (VISA or MC) information to: NAME, PO Box 69, Carmel, IN 46082-0069
  2. Fax it to the NAME office Fax: (317) 571-8105
  3. Join online via our secure shopping cart.

    To add an item to the shopping cart, click on the shopping cart icon on the right side.

    Once you are ready to check out, the website will be directed to a secure shopping cart site. (PLEASE NOTE: the https does not show up on all browsers navigation band but does show along the bottom of the screen as it is being redirected. Be assured that the site is secure and has all the proper encryptions in place.
